Scheduling, forms, records, and patient communications are handled through the secure OptiMantra platform. Information submitted through that platform is subject to its privacy and security practices as well as Signature Health's applicable patient privacy policies.
Basic technical information may be processed when you visit this website, such as browser type, device information, and pages viewed. Signature Health does not sell patient medical information.
General Information Only
Website content is educational and is not a diagnosis, treatment plan, prescription, or substitute for individualized medical care. Using this website, sending an email, or requesting an appointment does not by itself create a provider-patient relationship. A clinical relationship begins only after required registration, consent, and clinical acceptance steps are completed.
Services, membership benefits, appointment availability, and prices may change. Medications, laboratory testing, imaging, procedures, pharmacy charges, and care from outside facilities are separate unless specifically stated. Prescriptions and particular treatments are never guaranteed and depend on clinical evaluation and applicable law.
Location and Emergency Care
Telehealth care is available only when the patient is physically located in a state where Lilly is actively licensed and the requested service is permitted. Signature Health currently welcomes eligible patients located in Oklahoma, New Mexico, Kansas, and Colorado.
Telehealth has limitations. Some symptoms require an in-person examination, testing, urgent care, or emergency evaluation. Signature Health may recommend a higher or different level of care when medically appropriate.
Do not use website forms, email, portal messages, or routine appointments for emergencies. Call 911 or go to the nearest emergency department for severe or life-threatening symptoms. Secure messages are not monitored continuously.
